New Patient Registration

appointment_1When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.  Please make sure you bring with you your medical card along with 2 forms of identification. This should be a passport or photo driving licence and utility bill with your current address on.  If you do not have a medical card the receptionist will provide you with a form to complete. 

After successful registration you will be offered a short medical check up and appointment with our Health Care Assistant or Practice Nurse, this is to inform us about your previous medical history when you join the practice.

When you register you will also be asked to fill out a medical questionnaire.This is because it can take a considerable time for us to receive your medical records.

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice. Please do not forget your medical card and 2 forms of ID.
